Background
Multiple sclerosis (MS) is associated with persistent fatigue, impaired functional performance, reduced mobility, and decreased quality of life. Exercise is increasingly recommended as an important component of MS rehabilitation; however, the comparative effects of aerobic and resistance exercise remain unclear. This study aimed to compare the effects of aerobic and resistance exercise on fatigue and functional performance in individuals with MS.
Methods
A randomized controlled trial design was used to compare three groups: aerobic exercise, resistance exercise, and control. Participants completed a 12-week supervised exercise intervention. Fatigue was assessed using the Modified Fatigue Impact Scale (MFIS), while functional performance was evaluated using balance, mobility, walking endurance, strength, and aerobic capacity measures. Between-group differences and intervention effects were analyzed.
Results
Both aerobic and resistance exercise groups demonstrated significant improvements compared with the control group. Aerobic exercise produced greater reductions in fatigue and improvements in walking endurance and aerobic capacity. Resistance exercise resulted in superior improvements in balance and muscular strength. Functional mobility and quality-of-life outcomes improved following both exercise approaches, although the magnitude of change varied according to the specific outcome. Large effect sizes were observed for both exercise interventions compared with control.
Conclusion
Aerobic and resistance exercise are effective approaches for improving clinically important outcomes in individuals with MS. The findings indicate that exercise selection should be individualized according to rehabilitation goals, with aerobic training emphasizing endurance and fatigue management and resistance training targeting strength and balance impairments. These results support the use of structured exercise programs as an essential component of comprehensive MS rehabilitation.
